Story Medical Clinic Offers Free Sports Physicals to Area School-Aged Athletes
NEVADA, IOWA – With the aim of ensuring the state’s junior high and high schoolers are healthy enough to hit the field (or court, pool, course, etc.), the state requires sports physicals to be completed before athletes are cleared for competition on school-affiliated teams.
Story Medical Clinics are proactive in promoting these sports physicals and are offering them free from 2:30 to 5 p.m. on Wednesday, July 20 at the Nevada and Maxwell Clinics, and Monday, August 1 at the Nevada Clinic only. The free sports physicals are open to all area school-aged athletes. No appointment is necessary.
Attendees should download the physical form required by the state athletic associations at www.storymedical.org/sportsphysical and bring it with them. The team at Story Medical Clinic will complete the form at the appointment.
To compete on a school-sponsored team, the form must be completed and signed by a licensed physician, osteopathic physician, osteopath, advanced registered nurse practitioner (ARNP), physician assistant or qualified doctor of chiropractic, stating the student-athlete may safely engage in athletic competition. The form must be completed annually.
