NEVADA, IOWA – With the aim of ensuring the state’s junior high and high schoolers are healthy enough to hit the field (or court, pool, course, etc.), the state requires sports physicals to be completed before athletes are cleared for competition on school-affiliated teams.
Story Medical is proactive in promoting these sports physicals and is offering them free from 2 to 5 p.m. on Tuesday, June 16 and Tuesday, August 4. The free sports physicals will be offered at Story Medical Clinic in Nevada and are open to all area school-aged athletes including those at schools in Story and Marshall counties.
Precautions are being taken in response to the COVID-19 pandemic. All student athletes will be required to wear a mask throughout. If student athletes don’t have a mask of their own, they will be provided one upon entering the clinic. Only the student athlete receiving the physical will be allowed to enter the clinic.
No appointment is necessary. All student athletes should download the sports physical form required by the state’s athletic associations available at www.storymedical.org/sportsphysical and bring it with them. The team at Story Medical Clinic will complete the form at the appointment.
To compete on a school-sponsored team, this form must be completed each year. It was developed with the assistance of the Committee on Sports Medicine of the Iowa Medical Society and has been approved for use by the Iowa Department of Education, Iowa High School Athletic Association, and Iowa Girls High School Athletic Union.
The form must be completed and signed by a licensed physician, osteopathic physician, osteopath, advanced registered nurse practitioner (ARNP), physician assistant or qualified doctor of chiropractic, stating the student-athlete may safely engage in athletic competition.
