Directions

From Highway 30

Story County Medical Center - South Campus, is located .1 miles south of Hwy 30 on South 19th Street in Nevada. It is located at the corner of South 19th Street and G Avenue.

Story County Medical Center - North Campus, is located .7 miles north of Hwy 30 on 6th Street in Nevada. It is located on the west side of 6th Street between F Avenue and G Avenue.

Parking

Parking is located directly across from the main entrance at both campuses and is free to all patients and visitors. There is a circle drive at both campuses available for drop off/pick up of any patient, resident or visitor who needs easy access to the main entrance.

Main Entrances

The main entrance of the South Campus is located on the east side of the campus off South 19th Street. This entrance should be used to access all areas of the South Campus except the emergency room and receiving.

Story Medical’s Emergency Entrance is located at the South Campus. The Emergency Entrance is off S G. Avenue on the north side of the medical center.

All semis should use the entrance off of South G. Avenue. No truck traffic is allowed in the east parking lot area.

The main entrance of the North Campus and Senior Care is located on the south side of the North Campus off F Avenue. This entrance should be used to access all areas of the North Campus except Story Medical Clinic – Nevada and 21st Century Rehab, which have their own entrances.

21st Century Rehab

21st Century Rehab is also located on the North Campus. Its entrance is on the west side of the building off of 5th Street at 519 F. Avenue. Parking for rehab patients is located on 5th Street on the west side of the building.

Story Medical Clinic - Nevada

Story Medical Clinic - Nevada is located on the southwest corner of North Campus at intersection of F Avenue and 5th Street in Nevada at 519 F. Avenue. Its entrance is on the east side of the clinic. Story Medical Clinic - Nevada parking is located on F Avenue on the south side of the clinic.

Story Medical Clinic - Zearing

Story Medical Clinic - Zearing is located at 112 West Main Street in Zearing. Parking is located on West Main Street.

Story Medical Clinic - Maxwell

Story Medical Clinic - Maxwell is located at 403 First Street in Maxwell. Parking is located on First Street.

Story Medical Clinic - Slater

Story Medical Clinic - Slater is located at intersection of Hwy 210 and Linn Street in Slater at 600 Linn Street. Its entrance is on the west side of the clinic at Suite B. Story Medical Clinic - Slater parking is located to the west of the clinic. 

Story Medical Clinic - Natural Health

Story Medical Clinic - Natural Health is located at 431 South Duff Avenue in Ames. Its entrance is on the east side of the clinic at Suite B. Look for the red awning; the clinic is set back from South Duff Avenue behind Car X. Story Medical Clinic - Natural Health parking is located to the west of the clinic.
